COVID 19 SAFE SPA CLIENT FORM AND PROTOCOL
In an effort to keep our patients and team members safe, everyone must follow our MANDATORY COVID-19 PROTOCOL and fill out the client form. This form is to be used as an individual client record for guest each time they have a scheduled appointment. It is required to ensure that during the COVID-19 Pandemic, there are no symptomatic signs of the virus during the service visit. We ask all patients who are feeling ill to stay home and reschedule their appointment in order to protect other clients and team members.
PROTOCOL
1.
You will be ask to wear a mask during your visit. Mask will be provided if you don’t have one.

Wash your hands after your service.

Sanitize your hands with the hand sanitizer provided when entering the facility.

Sanitize your hands with hand sanitizer before leaving the facility.
